How do I check eligibility and verify benefits?

You can check eligibility a few different ways.

2) Chat with us online at https://sanabenefits.com/.

Please be sure to have the following information ready:

  • Patient Name
  • Patient Date of Birth
  • Member ID Number
  • Provider office NPI number

3) For a pre-authorization, please call (877) 608-2200 to speak with our care management partner, INETICO. They handle all pre-certifications and utilization management.
