Dependents automatically lose coverage at the end of the month of their 26th birthday. The exception to this rule is dependents who are mentally or physically unable to earn a living.
To continue coverage for a disabled dependent, the plan subscriber must contact Sana with the following documentation.
- Statement from the subscriber stating that dependent is unable to support themself financially and is reliant upon the subscriber
- Written documentation from the eligible dependent's treating physician or psychiatrist clearly identifying that the disability exists and that the dependent is mentally or physically incapable of self-sustaining employment
