Disabled Dependents Over 26

Dependents automatically lose coverage at the end of the month of their 26th birthday. The exception to this rule is dependents who are mentally or physically unable to earn a living.

In order to continue coverage for a disabled dependent, the plan subscriber must contact Sana with the following documentation. 

Required Documentation

  • Statement from the subscriber stating that dependent is unable to support himself/herself financially and is reliant upon the subscriber
  • Written documentation from the eligible dependent's treating physician or psychiatrist clearly identifying that the disability exists and that the dependent is mentally or physically incapable of self-sustaining employment 
Once the subscriber emails this documentation to hello@sanabenefits.com we'll verify the disability and get back to you within 3-5 business days.
