Case Management

Sana's Case Management program offers additional support to manage chronic health conditions, ensuring you get the care you need when you need it. 

Case Managers are highly-trained to:

  • Help you understand and manage your health conditions.
  • Help you understand and get the most from your benefits, including access to care, expert second opinions, equipment, medications, and additional services.
  • Help you coordinate treatment plans and streamline medical team communication.
  • And more!

If selected for this program, you may be assigned to Sana's in-house Case Management team or Valenz Health Case Managers, based on need and specialty. Your Nurse Case Manager will support and advocate for you as you move through your health care journey. You'll meet regularly (monthly or bi-monthly) to review your progress and update your care plan. 

This program is available at no cost to you, and you can opt out at any time.
